The information that is outlined above will be used for the following purposes: To conduct a psychological assessment To inform the development of an individualized intervention plan To maintain accurate clinical records To provide invoices and complete payments To communicate regarding appointments, ongoing support through follow ups Limitations of Confidentiality There are certain instances when information must be shared with third parties without your explicit consent: For the purpose of legal obligations e.g., risk of harm to you or others, ordered by the court

Keeping Your Information Secure

Ensuring that keep your personal information is a top priority therefore the following is conducted: Client personal information will be securely kept on an encrypted device All telehealth sessions will be conducted on a secure electronic video conferencing system

Your Rights to Your Information

You have the right to: Request access to the personal information collected Correct or update any inaccurate information Request for personal information to be deleted or restricted; please note this will be subject to legal and ethical obligations Withdraw your consent at anytime

Your Information: Retention, Disposal and Breach Notification

Your personal information will be held no longer than necessary. This will be in accordance with professional and legal standards. When necessary, your personal information will be securely deleted/destroyed. In the unlikely event a data breach occurs posing a risk to your personal information you will be notified and the Office of the Privacy Commissioner in accordance with legislation.
